The LA16 – Collision Reporting and Evaluation Procedure is a protocol set up between Local Authorities and An Garda Síochána with support from the Department of Transport, the CCMA, the LGMA and TII for the collection of data at fatal collision sites. The data collected is for statistical purposes only and does not form any part of the Forensic Collision Investigation.

The completion of LA16 forms for at least 70% of fatal collisions is an Action (no61) of the Road Safety Strategy 2021-2030 - PHASE 1 ACTION PLAN 2021-2024
